You decide:

Who your primary physician will be. This physician will coordinate all of your health care needs
When to see a specialist — no referrals needed
If you choose a network physician, it is their responsibility to submit claims
If you choose a non-network physician, it is your responsibility to submit claims, and your out-of-pocket costs will be higher
