Health Plans

A UnitedHealthcare medical plan gives you the freedom to see any doctor or hospital in our nationwide network, including specialists, without a referral. You can locate network doctors easily at myuhc.com®.

Note: Not all of the health plans listed will be available to you. Please see your employer benefit information to learn which of the plan(s) are available to you.

Choice Plan

This plan includes copayments, coinsurance and deductibles. You have the freedom to see any doctor, including specialists, hospital or pharmacy in our national network. You don’t need referrals and you don’t need to choose a primary care physician. If you receive care outside of our network, this plan will not cover your costs (unless it is a qualified emergency).

Options PPO Plan

This plan includes copayments, coinsurance and deductibles. You can see any doctor, including specialists, hospital or pharmacy in our national network. You don’t need referrals and you don’t need to choose a primary care physician. If you choose to receive care outside of our network, we have you covered but your costs may be higher. Your plan may be called a “non-differential” PPO Plan. This just means that if a network doctor is out-of-area, you can see a non-network doctor and receive the network rate.

HRA Plan

The HRA Plan has an annual deductible. The plan also includes a spending account called a health reimbursement account, or HRA. The employer funds the HRA. This money helps pay for eligible health care expenses, which apply to the deductible. In this plan, you can see any doctor, including specialists, hospital or pharmacy in our national network. You don’t need referrals and you don’t need to choose a primary care physician. If you choose to receive care outside of our network, we have you covered but your costs may be higher.

HSA Plan

The HSA Plan has an annual deductible. The plan lets you open and deposit money, tax free, into a personal bank account called a health savings account, or HSA. You can use the money to pay for qualified medical expenses, including your annual deductible, or save it for future expenses, even for retirement. The HSA also has tax benefits. In this plan, you can see any doctor, including specialists, hospital or pharmacy in our national network. You don’t need referrals and you don’t need to choose a primary care physician. If you choose to receive care outside of our network, we have you covered but your costs may be higher.

Navigate Plan

When you enroll in a Navigate Plan, you get a health plan that puts you on the path to the quality of care you deserve. And you get the protection you need from major health care costs.

The plan requires you to choose a primary care physician. Your primary care physician will be your first contact, who will work with you through all of your health care needs. They will guide you toward the right treatment, or the right specialist, at the right time.
